I have a few times, within a superhero campaign.

I've done it with adventures like The Pentacle Plot (where the heroes keep their powers) and in an environment where the heroes lose their superpowers, making it much more challenging for them.

I have a D&D/Pathfinder campaign world and a V&V world, and the two are just a dimensional rift apart from each other. So, yes, the their are earthlings in my fantasy world, and pathfinders in my V&V world.
