I've run campaigns without a designated leaders. The heroes just kind of work things out themselves. What would be a situation when you'd need an official leader? Or rather, when would not having a specific leader hurt the party?
I can think of quite a few, actually.
If the ground is in a really intense battle, especially if one or more members have fallen (and are at risk of being captured). It takes a leader to decide when it would be appropriate (or strategically best) to have the other members flee. Sometimes it's better to regroup and reattack.
When the group is split on what to do (three want to do one thing, and four want to do another).
When an inspiring, charismatic type is needed to persuade someone, or perform an oratory for a group, or rally the troops.
We have had times where we have had multiple leaders before. Where there might be one overall leader for the team, but another is the combat (tactical) leader, and yet another might serve as the charismatic/speech/persuasion leader-type.
Overall I think a leader is needed because there are often split-second decisions to be made (where the fate of the world might hang in the balance), and there's just plain not the time or ability to make the decision in a committee.
